For Pre-certification: Call (866) 857-5211, you must call 7 days before any inpatient hospitalization or confinement, or home health care / hospice service or before outpatient surgical or diagnostic procedures.  Notify us immediately upon a diagnosis of pregnancy. Emergency & Maternity admission must call the next business day. 

Medications on the Specialty Drug List require pre-certification at (602) 906-6253.  Please click here to review the specialty drug list . Penalties will apply if you fail to pre-certify.
